18 Weeks Case Studies - 18 Weeks Objective
Department of Health policy is that by December 2008 patients should wait no more than 18 weeks from GP referral to treatment. South Devon Health Community has been selected to achieve this a year early: by December 2007. Case studies show how we are achieving this.
Across the health community, we are trying to achieve by developing new Integrated Care Pathways (ICPs).
The following diagram demonstrates a typical ICP:
This is a typical ‘integrated care pathway’ or patient journey that covers your total experience. As you can see this is from the moment that you recognise that something is wrong to the time that you return home treated or ‘feeling better’. Within the CITEC programme, we have developed a portfolio or a group of projects. Some of these projects have successfully re-designed the total ICP. Others have re-designed parts of the ICP.
